Hospice PBM FAQ
Hospice pharmacy benefits, explained plainly.
A hospice PBM manages prescription drug costs, the formulary and pharmacy network for hospice organizations. These answers explain how MaxHaven Rx prices drugs, measures PPD, supports Medicare Hospice Benefit compliance and structures contracts, using only what MaxHaven publishes about its own service.
About MaxHaven
What is MaxHaven Rx?
MaxHaven Rx is a hospice-specialized pharmacy benefit manager (PBM) launched in 2023 by PPOk. It manages prescription drug costs and formulary compliance for hospice organizations: for-profit, non-profit, hospital-based and home-health-based programs. It works on a transparent, no-spread-pricing model with a formulary designed by palliative care pharmacists.
Who owns MaxHaven Rx?
MaxHaven Rx is part of PPOk (Pharmacy Providers of Oklahoma), a family of pharmacy brands founded in Oklahoma in 1985 by independent community pharmacists. MaxHaven launched in 2023 and is a sister brand of MaxCare Rx, PPOk’s pass-through PBM for employers.

Pricing and savings
How does MaxHaven reduce hospice pharmacy costs?
Three ways. First, it benchmarks drug prices to NADAC, what pharmacies actually pay, instead of inflated AWP list prices. Second, it eliminates spread pricing and hidden markups, so you pay the actual drug cost plus a clear dispensing fee. Third, its hospice-specific formulary is optimized to reduce PPD without compromising care; formulary optimization drives 50–70% of total client savings.
What is PPD in hospice pharmacy management?
PPD stands for per-patient-per-day drug cost, the main metric used to benchmark hospice pharmacy spend. It is the average daily cost of all medications dispensed to a hospice patient. MaxHaven’s average PPD is less than $6.00, compared with an industry average of $11.00, and MaxHaven provides real-time PPD tracking for every client.
Will MaxHaven benchmark our current PPD before we sign?
Yes. MaxHaven will benchmark your current per-patient-per-day drug costs against industry standards at no cost and with no commitment, before any agreement is signed.
What is NADAC and why does it matter for hospice?
NADAC (National Average Drug Acquisition Cost) is a public pricing benchmark published by CMS that reflects what retail pharmacies actually pay to acquire drugs. Pricing tied to NADAC is easier to verify than pricing tied to AWP (Average Wholesale Price), a list price that usually sits well above acquisition cost.
What is spread pricing, and does MaxHaven use it?
Spread pricing is when a PBM charges a plan more for a drug than it pays the pharmacy and keeps the difference. MaxHaven does not use spread pricing. You pay the actual cost of the drug plus a clear pharmacy dispensing fee.
Clinical and compliance
What is the Medicare Hospice Benefit, and how does MaxHaven support compliance?
The Medicare Hospice Benefit (MHB) covers medications related to a patient’s terminal diagnosis under the hospice per diem rate, so the hospice is financially responsible for those drug costs. MaxHaven’s formulary and compliance protocols are built around MHB requirements, with built-in guardrails and audit support for CMS regulations.
Who designed the MaxHaven formulary?
Palliative care pharmacists who understand the Medicare Hospice Benefit. It was curated by a clinical pharmacist with more than a decade of specialization in managing hospice specialty pharmacies.
Does MaxHaven offer clinical support for hospice staff?
Yes. Hospices can work with a hospice clinical pharmacist on customized opioid conversions and educational in-services on hospice disease states. MaxHaven also trains staff to manage prior authorizations and eligibility, and offers customizable emergency kit (E-kit) management.
Working with MaxHaven
What types of hospice organizations does MaxHaven serve?
Hospice administrators and executive teams, hospice consortia and group purchasing organizations, non-profit hospices, for-profit hospices, hospital-based hospice programs and hospice start-ups. The contract structure adapts to size, ownership model and patient population.
How are MaxHaven contracts structured?
A base contract with add-ons available à la carte, so you pay only for the technology you need. The structure works for non-profits, for-profits, hospital-based programs, established hospices and start-ups.
Does MaxHaven integrate with our EMR?
MaxHaven offers e-prescribing and integration with major hospice EMR platforms, which reduces double entry for clinical staff and the risk of errors in patient demographic data.
